Richmond Item, December 11, 1903, p. 6 - The funeral of Perry C. Hazeltine will be at the home, 211 Randolph street, Sunday afternoon at 1 o'clock. Friends may call at any time. The burial will be at New Paris.

Richmond Item, December 11, 1903, p. 1 - The death of Perry Hazeltine was announced in The Item yesterday. He was one of the best known and most highly esteemed young men of Richmond. His sickness dated back a long period and it had been known for some time to family and friends that recovery was impossible. He was member of Hinton lodge of Elks, at Hinton WV and of the Knights of Pythias in this city. [mentions pall bearers, burial will be at New Paris, the former home of Mr. Hazeltine.]
